How painful is Thermage FLX? Most people find it tolerable rather than genuinely painful. The sensation is a deep warmth that builds and releases with each pulse, and the FLX-generation handpiece adds vibration and surface cooling that scatter the nerve signal. Discomfort is highest where bone sits close to the skin, such as along the jaw.
Search for how painful is Thermage and you will find two contradictory accounts of the same treatment: one describing a relaxed hour, another describing gritted teeth.
Both are true, and the difference usually comes down to the device generation, the area treated, and how the operator paces the session.
What follows separates the sensation itself from the questions that sit right behind it. When results actually show up, how long they hold before maintenance, and whether Thermage is doing the same job as Ultherapy at all.
A word on who is writing. Relieve Clinic sits one minute from Exit 3 of Apgujeong Station in Seoul. Dr. Park Tae-oh, fifteen-plus years and more than 30,000 clinical cases, runs every consultation and every session himself in a private room. Only two or three patients are booked per hour, which is the operational reason a session can be paced to the patient rather than to the schedule.
What the Thermage Sensation Actually Is
Thermage FLX is a monopolar radiofrequency device. Rather than focusing energy to a single pinpoint, it heats a broad volume of tissue evenly, from the dermis down into the upper subcutaneous fat. That is why the sensation arrives as a spreading warmth instead of a sharp point.
Each pulse follows a short cycle. The tip cools the skin surface, energy passes, warmth peaks, the tip cools again. Patients often describe the peak of a pulse as a hot flush that fades before they have finished registering it.
Many describe the rhythm as almost hypnotic across the cheeks and forehead, then distinctly sharper along the jawline, where there is little fat cushion between the tip and the bone.
Nothing here is meant to be endured in silence. Energy levels are adjustable pulse by pulse, and telling the doctor mid-pass is the normal, expected thing to do. The protocol is designed to minimise discomfort, not to test tolerance.
What the FLX Generation Changed About Comfort
How painful is Thermage also depends on which generation of the platform you are reading about. If someone told you it was brutal, ask them when they had it.
Older accounts describe an earlier generation of the platform. FLX brought two changes that matter directly to how a session feels.
- A larger tip: more area covered per pulse means fewer total pulses for the same coverage, so the session is shorter and there are fewer peaks to sit through.
- A vibrating, cooled handpiece: vibration and surface cooling arrive at the nerve endings alongside the heat, and the competing signals dilute how sharply the warmth registers. This is the single biggest reason most patients now call FLX warm and tolerable.
- Pacing by the operator: a rushed pass concentrates pulses; a measured one lets each area settle. This is a human variable, not a device one.
The manufacturer’s own product information for the platform is published by Solta Medical, and it is worth skimming the maker’s description before a consultation so you can ask sharper questions.
Comfort settled, the next question is the one people actually book for.
When Thermage FLX Results Appear and How Long They Hold
Thermage works on two clocks at once. Existing collagen contracts immediately under the heat, which is why a subtle change is sometimes visible on the day.
Then new collagen forms gradually over roughly two to three months, and that slower process is what produces the change people describe as looking rested rather than treated.
| Stage | Timing | What is typically noticed |
| Immediate | Day 0 | Mild flushing; existing collagen contracts, sometimes visible as slight tightening |
| Settling | First week | Flushing resolves; the treated area feels normal again |
| Build | Weeks 4 to 8 | Texture and firmness shift gradually as new collagen forms |
| Peak | Months 2 to 3 | The fullest expression of the result |
| Maintenance window | After the peak | Held for a period that varies by age, skin quality and lifestyle before a repeat is considered |
That last row is where honest answers get uncomfortable, so it deserves its own section.

Does Thermage Last Forever?
No. Nothing in the non-surgical tightening category is a lifelong fix, and any clinic implying otherwise is overselling. Collagen keeps turning over, ageing keeps going, and the tissue you treated at thirty-five is not the tissue you will have at forty.
What Thermage does is reset the position of a moving baseline.
The result holds for a meaningful period after the peak, then gradually drifts, and most patients plan a repeat when that drift becomes noticeable to them rather than on a fixed calendar. Someone with thicker, well-supported skin will typically get a longer run than someone whose laxity is already advanced. Sun exposure, smoking and weight fluctuation all shorten it.
If your laxity followed a significant change in body weight, the mechanics are different enough that we have covered them separately in our guide to skin laxity after weight loss in Korea.
💡 If you are also weighing Thermage FLX cost in Korea for the same trip, that guide covers the part this one does not.
There is also a practical point about where the session lands in a trip. Flushing aside, Thermage leaves no crust, no peeling and no dressing, so it sits comfortably early in an itinerary. What it does not do is deliver its verdict while you are still in Korea.
You will fly home before the interesting part happens, which is why remote follow-up matters more for this treatment than for something with a visible recovery you can watch in a hotel mirror. Relieve keeps that channel open after departure, so the weeks after your flight are not spent interpreting your own skin, and an English coordinator stays reachable.
Thermage Versus Ultherapy: Different Jobs, Not Rivals
Patients arrive asking which of the two to pick, as though one must win. They target different layers, and a jawline can need both for different reasons.
Thermage heats a broad volume from the dermis into the upper fat, which suits surface tightening, fine lines, pore appearance and overall elasticity. Ultherapy focuses ultrasound to set depths of 1.5, 3.0 and 4.5mm, with the deepest reaching the SMAS, which is structural lifting.
| Platform | Energy | What it suits | Sensation |
| Thermage FLX | Monopolar radiofrequency, broad even heating | Surface tightening, fine lines, pores, elasticity | Warm and tolerable; vibrating cooled tip |
| Ultherapy Prime | Micro-focused ultrasound, 1.5 / 3.0 / 4.5mm | Structural lift; 4.5mm reaches the SMAS | Warm prickling per shot, gone in a second or two |
| Onda | 2.45GHz microwave (Coolwaves) | Contouring where deep fat is part of the problem | Warm massage; numbing usually skipped |
| XERF | Radiofrequency | Another radiofrequency option on the same clinic menu | Settings adjusted during the session |
A fuller side-by-side sits in our comparison of Ultherapy vs Thermage in Korea. For how radiofrequency sessions in general are quoted and what drives the figure, see our note on RF skin tightening cost.
One eyelid-specific caution worth carrying into any consultation: on thin eyelid skin, only the shallower ultrasound depths are appropriate. The 4.5mm depth is not used there. If a clinic offers it on the eyelid, that is a question worth asking twice.
Comfort Options to Discuss Before You Book
There is more to manage sensation than simply enduring it. Ask about these at consultation rather than on the treatment bed.
- Numbing time, not numbing product: the same cream left on for ten minutes and for a properly timed thirty to fifty minutes are two different experiences. High-volume clinics cut this step because it costs them room time.
- Energy titration: starting lower on the jawline and building, rather than running one setting across the whole face.
- Sedation: available for patients with genuinely low tolerance, discussed in advance rather than improvised.
- Who holds the handpiece: at Relieve, the doctor who consults is the doctor who treats, and the 3D skin analysis taken at consultation lets the same zone be compared as measurement at the next visit.
Two further habits shape how a session feels and neither is device-related. The first is hydration and sleep the night before; a tired, dehydrated patient reports heat more sharply.
The second is caffeine on the morning of, which raises baseline tension for some people. Neither is a rule, and neither changes the outcome, but both come up often enough in the treatment room to be worth mentioning.
On cost, one thing changed recently that affects every international patient. Since 1 January 2026 the VAT refund on aesthetic treatment for foreign patients in Korea has ended.
Whatever figure you are quoted, plus 10% VAT, is what you actually pay, and there is no claim to make at the airport on the way out.
This page is general information and not a diagnosis. Individual results vary, side effects are uncommon and are explained beforehand, and whether this platform suits your skin can only be judged in an in-person consultation with a licensed doctor.

How painful is Thermage FLX compared with Ultherapy?
They feel different rather than one simply hurting more. Thermage delivers a spreading warmth per pulse with a vibrating cooled tip, while Ultherapy gives a brief warm prickling at each shot. Both are sharper along the jawline, where bone sits close to the skin.
How long do Thermage results last?
The result peaks around two to three months after the session and then holds for a period that varies with age, skin quality and lifestyle before a repeat is considered. Nothing in this category is a lifelong fix.
Is there any downtime after Thermage FLX in Korea?
Mild flushing on the day is common and usually settles quickly. Most international patients continue their itinerary the same afternoon, though your doctor will confirm this for your skin at consultation.

Dr. Park Tae-oh is the founder and director of Relieve Clinic Seoul in Apgujeong, Gangnam, where he performs consultation and treatment personally. His work centers on reducing discomfort during aesthetic dermatology, drawing on more than 30,000 clinical cases. In June 2026 he demonstrated periorbital radiofrequency technique using XERF at Cynosure Lutronic Academy Korea.
