A “retracted” columella sits too high, causing the nostrils to appear pinched. A “hanging” or “ptotic” columella sits too low, making the tip look drooping.

Surgical Options for the Columella In Nose
Columellar Strut Graft
The gold standard for correcting a retracted columella. A piece of cartilage (usually septal or ear) is placed between the medial crura to rebuild structural support and length. Results are immediate and permanent.
Extended Spreader Graft
In more severe retraction—especially after prior rhinoplasty—surgeons combine a columellar strut graft with an extended spreader graft to rebuild the entire frame of the nose. This is typically a revision-level procedure.
Columella Reduction (for ptosis)
A hanging columella is addressed by trimming the caudal septum and repositioning the soft tissue. Shorter recovery than a full rhinoplasty, but still a surgical procedure under anesthesia.
